Individual Registration

To start the membership sign-up process, please fill out the following form. Once the form has been filled out, we will reach out to you for your payment method.

PAYMENT TERMS OF AGREEMENT FOR SENIOR LEARNING NETWORK SUBSCRIPTION
Billing/Payment Terms Effective August 2020

New Members: First Month’s subscription: ($10.00) due; payable by credit card.  Service will be activated for the following month once payment is received.  If a new member desires to be active for the present month, they will be billed for both the present and next month.

Monthly Payments: Monthly payments will only be available by credit card via automatic payment, ($10.00); deducted on the 15th of the month for the following month's programs.

Payment by check will only be accepted for an annual membership payment. If you wish to pay annually please check the box above.

Annual payments, $120.00.

Once this form is submitted we will reach out to you to set up your payment method.
