Basic or Premium Membership Registration

To start the membership sign-up process, please fill out the following form. Once the form has been filled out, we will reach out to you for your payment method.

PAYMENT INFORMATION: Membership is a monthly subscription.  If you would prefer to be billed annually for a discounted price,  please contact us. You will be invoiced the 30th of the month for the following month. You can be automatically charged on a credit card, or you may pay by check from your business.  Once this form is submitted we will reach out to you for your payment method. If you would like to speak with us about alternate payment methods, please just email us and we can discuss it.

Note: Premium Members that request a private program: If there is a fee charged by that provider, you will be invoiced for that program by the provider directly, not by Senior Learning Network.
